Silent Hill Townfall Pharmacy Puzzle: Full Step-By-Step Guide

Silent Hill Townfall drops players into one of its earliest brain teasers at the St. Amelia pharmacy in the Shorefront District, and it trips up many first-time visitors. Cracking it means reaching the building, waking up a hidden broadcast on your CRTV handheld, and then hunting down a handful of posters scattered around the store to piece together a four-digit locker combination.

Getting Into the Pharmacy and Activating the CRTV Signal in Silent Hill Townfall

Before you can even attempt the puzzle, you’ll need the Scalpel, which is picked up inside the nearby Family Clinic. Zoe’s Lanyard is what gets you through the clinic’s badge-locked front entrance in the first place; once inside, the Scalpel is waiting in an office down the corridor.

The Scalpel obtained at Zoe’s clinic is your ticket into St. Amelia’s pharmacy, and it’s here, specifically in a back office, that a blue, four-digit combination locker awaits. At the pharmacy’s entrance, examine the door and select the prompt to carve through it with your Scalpel.

Once you’re inside, make your way toward the rear of the building, close to the restroom, where a desk holds a radio. Interacting with it lets you pull out your CRTV and dial in its signal; this is where the game’s Fine Tuning mechanic comes in, having you align a distorted picture and an RGB signal until everything locks into place.

After locking onto the frequency, your CRTV stores a short, static-warped clip made up of four separate images, each one pointing to a different poster somewhere in the pharmacy. The footage only shows you where to look, though; you still have to track down each poster yourself and read the actual number off it.

Decoding the Posters to Crack the Locker Code in Silent Hill Townfall

With the signal saved, it’s time to track down the physical posters the CRTV footage references. Each poster hides its digit a little differently; one has you counting something in the image, another has the number printed right on it, and so on:

• Digit 1: In the bathroom, count the fingers shown on the hand-washing poster.
• Digit 2: Head down the short staircase to the right of the front desk to find a second poster. The step count is printed directly on it, so you’ll need to go find it rather than catching it in the CRTV clip.
• Digit 3: Track down the blood-pressure machine poster near the front entrance; whichever number the arrow lands on is your third digit (it points to 3).
• Digit 4: In the back office, check the poster positioned near the lockers and tally how many pills remain in the strip; that count (9) is your final digit.

Put together, the full combination is 6539. Punch that into the keypad and the locker will open. Inside, you’ll find the Prescription Dosage Error Note, a key item. This item ties into a separate weighing puzzle you’ll tackle once you return to the Clinic, so it’s worth grabbing even if you’re speedrunning through the Shorefront section.

Solving the pharmacy locker directly feeds into a larger puzzle back at the clinic, so getting it right the first time saves you a return trip through the Shorefront’s fog-choked streets.
