His final confrontation with Muzan takes that endurance to another level, with Tanjiro continuing to fight despite catastrophic injuries and blood loss. He ranks fifth because his resilience is impressive, but the others have demonstrated either greater sustained trauma or more extreme psychological and physical punishment.
4 Jujutsu Kaisen‘s Yuji Takes Devastating Hits and Still Gets Back Up

He fights Choso, survives the aftermath of Sukuna’s rampage, witnesses devastating losses, and then faces Mahito while already physically and psychologically battered. Mahito repeatedly damages him, including landing Black Flash, yet Yuji eventually returns to the fight alongside Todo. His endurance is not just about taking punches: Yuji repeatedly absorbs emotional devastation without allowing it to permanently stop him.
Later in the manga, he also develops Reverse Cursed Technique and continues fighting through severe injuries in the final battle. He ranks fourth because his pain tolerance is remarkable, but his demonstrated history of sustained bodily trauma is still narrower than Kaneki’s, Luffy’s, or Guts’.
3 Kaneki Was Tortured Until Pain Became Part of His Survival

His transformation from a frightened college student into someone willing to embrace his ghoul nature in Tokyo Ghoul demonstrates extraordinary psychological endurance as well as physical resilience. He falls below Luffy because his greatest endurance breakthrough is concentrated around one horrific period, whereas Luffy repeatedly demonstrates his ability to fight through punishment across multiple arcs.

During Impel Down, Magellan’s poison leaves him near death, yet Luffy survives an agonizing treatment lasting roughly 20 hours and immediately resumes his mission to save Ace. At Marineford, he continues fighting after the punishment he has already endured in prison and repeatedly pushes his body beyond what others consider safe.
His endurance also has a distinctly Luffy-like quality: pain rarely changes his objective. The combination of catastrophic injuries, poison, and exhaustion makes his record exceptional, but Guts’ much longer history of surviving severe physical trauma gives the Black Swordsman the edge.

As a child, he endured horrific abuse before becoming a mercenary, and the Eclipse left him without his left arm and right eye while permanently marking him as a sacrifice. Yet Guts continued fighting apostles afterward, wielding the enormous Dragon Slayer and a prosthetic cannon.
His later use of the Berserker Armor pushes that endurance even further, allowing him to keep fighting while his body is being driven beyond its limits. What separates Guts from the rest is the sheer duration of his suffering: physical pain, repeated battlefield injuries, and trauma all become part of his continued survival.
