Shark Fin in Path of Exile 2 looks like a troll drop the first time you see it. It’s a big, inventory-hogging drop from the Great White One in Act 4. The Shark Fin refuses to leave your bag until you solve its mystery.
It eats your inventory until you figure out what to do with it. However, thanks to a recent update, it now also grants a permanent defensive bonus if you hand it in.
The game gives you almost no clues; this is where we come in! So, here’s how to use Shark Fin in Path of Exile 2 without wandering the Karui islands forever.

You get the Shark Fin by defeating the optional boss Great White One in the Shark Pit area of Whakapanu Island during Act 4. Once it drops, it automatically starts the Tribal Medicine side quest (“Find a use for the Shark Fin”).
Keep in mind that you can’t just drop or stash the Shark Fin. Completing the quest is the only way to get rid of it. To finish Tribal Medicine, you need to bring the Shark Fin to Kaimana, a Karui healer tied to the islands south of Kingsmarch.
Here’s how you can get to Kaimana:
- Portal or travel back to Kingsmarch after looting the fin
- Talk to Makoru on your ship and choose Ngakanu as your destination
- When you dock, head through the starting village and look to the right side to find Kaimana with a quest marker
Earlier, Kaimana’s location could shift later in the act depending on how far you’d pushed the main story. That sometimes moved her onto the tribal ship in Kingsmarch. That behavior is still prevalent after the 0.5 patch.
If she isn’t in the Ngakanu village, check the bridge of the Karui ship in Kingsmarch instead. As soon as you talk to her with the Shark Fin in your inventory, she recognizes it. Kaimana will then take it for her medicine experiments and present you with the new, buffed reward options.

Hence, it was easy to skip if you weren’t desperate for Act 4 gems. As of patch 0.5, Grinding Gear Games reworked Tribal Medicine.
The fin now grants a permanent defensive passive instead, putting it firmly in the do not skip category. Presently, when you hand the fin to Kaimana, you now choose between two permanent options:
- Kaom’s Lesson (Red) – Grants 30% increased Armor, Evasion, and Energy Shield
- Rakiata’s Lesson (Blue) – 15% of Armor also applies to Elemental Damage, Gain Deflection Rating equal to 12% of Evasion Rating, and 12% faster start of Energy Shield Recharge
Both rewards are permanent, character-wide passives. They don’t sit on gear, they don’t disappear on respec, and they apply for the rest of that exile’s run. GGG has truly redeemed itself with this update, unlike their previous ones.
Because this quest is now one of the few ways to get a free, permanent defensive multiplier in Act 4, we warn against skipping it. Even if you’ve already finished the island’s main objectives and just want to move on.
